You will have to decide if you are going to fill out the trip request form or the have this done by your client.
If the client is going to make their own appointments, you'll have to give them the number for them to call.
You can call information or email in the completed form. MTC will verify with you that the trip has been set up. At the end of the month, billing statements will go out. It will show you what it is costing you for the rides. If you are a recipient of the grant funds or New Freedom Funds, it will be on the bill. You will pay nothing during the grant period. During that time, we will ask for your input for improvements in the service to make it sustainable for the future. If you are paying partially for the service, it will be reflected on the bill.
