Once the partners sign an agreement, MTC will work with the partners to make the process as smooth as possible.
Partners will be able to sign up clients as soon as you are ready.
You will need to fill out a Trip Request Form.
You have a choice to fill out the Trip Request Form or the client can call for a ride and we will work with the client if you spell out everything on the Parameters of Service. We will not deviate from Parameters of Service, if the client wants additional stops and you say no, we do not stop. If there is an end date to how long they can call for a ride, you need to put it on the parameters of Service. If your agency has different staff people filling out the forms, we suggest the staff person sign the bottom of the form with their name. If you fill out the trip request form and the client have re-occurring appointments, you can make them on the one trip request form.
At the end of the month, you will receive a bill for the rides provided. The bill will show the clients name, date of service, destination, cost of service. If you need any other information, we will work with you. Riders will be given a prepaid postcard for feedback from the Driver. They can mail the pre-‐paid card at their convenience.
We have New Freedom monies that will pay half the cost of the ride for people who are 60 years of age and older or people with disabilities. We will use most of this money for out of town trips since they are so much more expensive.
Please remember this is a work in progress! We depend on our providers and partners to help us improve the process! Each month the providers and partners will be notified to see if there are any concerns or questions.
